Catholic Health offers a variety of ways to access your health information and obtain your medical records any time, day or night. Additional information may be found by viewing an easy-to-read tool “Access to Health Information: Your Rights; Our Responsibilities.”
Catholic Health MyChart
Catholic Health MyChart allows you to immediately access and print your medical records anywhere, anytime. You can even share, link or print information from your account. If you do not have a Catholic Health MyChart account and wish to activate one you can do so at the link provided here or call 631-465-6100.
Requesting Medical Records – Online Portal
Catholic Health's online portal makes it easy to get a copy of your medical records. You can submit a request from your smartphone, tablet or computer, and your records will be delivered via email (preferably) or by mail.

Requesting Copies of Radiology Images
If you require copies of your radiology study/exam images, please complete the Authorization for Release of Health Information (above). Once complete, please send to the facility or entity where the study/exam was performed.

Deceased Patient Medical Record Updates
An immediate family member, executor or other person authorized to act on behalf of a patient, may submit a written request to update the record of a deceased patient. Documentation including the requestor’s ID as well as a copy of the patient’s death certificate may be required. You can complete the form below, print and send (with other documentation) to the facility, entity or practice where the patient was last treated.
